This practice participates in most insurance plans including PPO’s, EPO’s, POS’s, HMO’s and Medicare. We do not accept assignment on Medicare which means that the patient is requested to pay for the visit at the time of examination, an electronic claim will be forwarded to Medicare, and reimbursement will be sent directly to the patient by Medicare as well as your secondary coverage.
At the time of your appointment, please make sure we have your current information. Insurance cards are often updated more than once a year and policy numbers may change. Please show your card to the receptionist at each visit.
If you are enrolled in an insurance plan that requires a referral from another doctor, please make sure that we have that authorization so that there will be no delays in your treatment. Payment of “non-covered” services, such as copays, deductibles, refractions, contact lenses and other cosmetic treatments can be made in cash, Visa, MasterCard, American Express and Discover.
If you have coverage with a company with which we do not currently have a contract, you will be asked to pay for your visit at the time of exam. A receipt will be given to you so that you may send that to your carrier for possible reimbursement as an “out of network” exam.
